Agnesian Healthcare provided in-home Hospice Care for years and desired to build an independent facility to compliment their program. Individual rooms and bath rooms were researched and developed for patient care and maneuverability as well as privacy. Three pods of four patient rooms flow out from a center core which contains large Bathing Facility, Family Room, and a small Chapel. The main Entry flows into a Living Room, adjacent Dining Room, Kitchen, and Staff Office. Great care was taken to provide a home-like setting throughout the building, including extensive landscaping along the exterior walk around the site.  The project was completed with CD Smith as a design-build project.
